Thank you for your interest in being a Mentor for the LGBTQ+ Mentorship Program from the Center for Sexual and Gender Diversity. Mentors students who feels experienced in LGBTQ+ identity topics and are willing to facilitate conversation with a peer. Each mentor must fill out this intake form and then go through the interview and training process adequately. Mentors directly impact the lives of our Mentee students that would like guidance navigating areas of LGBTQ+ identity.

After completing this questionnaire, you may be contacted to partake in an interview to be a mentor for the LGBTQ+ Mentorship Program.
