Forms for F-1 Students

 

IMPORTANT UPDATE

The International Education Center is happy to unveil our new International Student Portal that you will use to submit required information such as the mandatory Check In Form for new/transfer students and request F-1 benefits such as reduced course loads, I-20 travel signatures, assistance with Optional Practical Training (OPT), and much more. 

Please Note: We will no longer use the F-1 student forms previously located on this website. All requests except for those still listed on this site must be initiated through this new portal. 

To login to the new portal, go to http://iec.smc.edu. Click on the blue Login button in the upper right corner, then choose "Log in with Campus ID". Use the same username and password that you use to login to Corsair Connect. Then click on the Requests & Documents tab and select the icon for the request or service that you need. In the future when you request a new I-20 or a letter of any kind, you will be able to find and download them directly from the Requests & Documents tab.

CSN Memorandum
Current students sponsored by CSN who need to verify Summer or Winter term enrollment. Students must enroll in their classes before submitting this form. The form must be completed and emailed to intlcounselor@smc.edu.

Health Insurance Refund Request Form
Students who had to withdraw from SMC after health insurance fees were paid. Must submit with required documents listed on the form.

SMC's Policy on Completing Status or Transfer Verification Forms for Other Schools

SMC will only complete SEVIS transfer requests for students who have maintained valid F-1 status and have an “ACTIVE” SEVIS record. SMC does not complete transfer or status verification form(s) from other institutions, as they are not a regulatory requirement for completing F-1 SEVIS transfers (8 C.F.R. § 214.2(f)(8)(ii)). Upon release, previous program dates, work authorization, and any additional information requested on the transfer verification form are accessible to the new school in SEVIS. It is the student’s responsibility to provide proof of prior work authorization, if requested.

Upon completion of the student’s SEVIS transfer request, the student will receive a confirmation email from intled@smc.edu. In lieu of a transfer verification form, the student may forward this email confirmation from SMC to their new school as evidence they have met eligibility requirements for a SEVIS transfer in “ACTIVE” status. If a student is NOT in "Active" status, they will be notified to inform their new school and an email from the new school is needed before a Completed or Terminated record will be released.
