The Centennial High School PTSA is pleased to announce our annual Senior Scholarship Program. Applications are now being accepted and four (4) $500.00 awards are available to graduating senior students from the class of 2023. Winners will be awarded scholarships at the CHS Senior Awards Night in May 2023. Community support of the PTSA and its programs make these scholarships possible. Application essays may be used by the PTSA to recognize CHS Mentors.

ELIGIBILITY CRITERIA

Applicants must...

1. Be a 2023 CHS graduating senior.

2. Be a 2022-2023 CHS PTSA student member, with at least one family member as a registered 2022-2023 CHS PTSA adult member. Membership fees must be paid by April 28, 2023. 

3. Have achieved a minimum cumulative unweighted G.P.A. of 2.0 through the first semester of senior year. 

4. Be accepted to a college (2 or 4 year), art school, trade/military school, or technical school.

REQUIRED TWO (2) ESSAYS
---------------------------

Type both essays in MICROSOFT WORD or ADOBE PDF file format
ESSAY 1:  (In 400 words or less) As you reflect on your four years of high school, describe how you have been a positive influence on your family, your community, and/or Centennial High School.

ESSAY 2: (In 100 words or less) Describe your most unique attribute/skill/quality that differentiates you from everyone else.

INSTRUCTIONS

STEP 1:
Please email your CHS counselor to access your student records. Your counselor will confirm that you are a student who meets the minimum GPA requirements, along with your post-high school status for scholarship eligibility.

STEP 2:
After you obtain email approval from your counselor to confirm your eligibility, please copy-paste your counselor’s confirmation email response to this online application form.

STEP 3:
Type your two (2) required essays in Microsoft Word or Adobe PDF file format. 
*** DO NOT INCLUDE ANY OF YOUR PERSONAL OR CONFIDENTIAL INFORMATION (such as your FIRST NAME, LAST NAME, NICKNAME, CONTACT INFORMATION, SOCIAL SECURITY NUMBER, ETC) in the contents of essay files.
*** NAME YOUR FILE as "Essay1" and "Essay2". FILE NAMES SHOULD NOT CONTAIN ANY IDENTIFICATION INFORMATION.

STEP 4:
Confirm that at least one family member and yourself have both joined CHS PTSA during school year of 2022-2023 and paid memberships dues by April 28, 2023.  To join CHS PTSA Membership, visit www.chs-ptsa.org (adult membership is $15 per adult, and student membership is $10 per student).

STEP 5:
Submit this online form by the deadline: Friday, April 28 at 5:00 PM EST.

STEP 6:
You will receive a confirmation email within five minutes after your submission of this online application.
