Connecting the Behavioral Health Ecosystem

We’ve designed a platform that can evolve with a changing healthcare landscape – a landscape that transcends traditional boundaries. With web-based tools that help teams collaborate across organizations, you can provide better care that costs less.

Implementing Chronic Care Management

bhworks includes tools that track care management services so providers and staff can work together to manage care plans, track referrals, engage patients, and evaluate outcomes. It even comes equipped with a built-in timer, notifying care managers when they can bill for CCM or BHI tasks.

Addressing Social Determinants of Health

Collaborate with community-based organizations to address barriers to care like access to employment, housing, transportation, food, and phone service. Real-time dashboards give you population-level information that can help your organization better allocate resources for value-based care programs, addressing issues that affect a patient’s health outcomes long after they leave your facility.

Meeting Joint Commission Standards

Our platform provides standard outcomes measurement that can help you meet new regulations for treating patients with behavioral health issues. bhworks can also operate inside or outside of the EHR, so you can leverage non-clinical staff to administer comprehensive assessments to screen for suicide risk and other behavioral health problems, sending alerts to designated clinicians when a safety plan is needed.

Leading By Example

Our clients have great stories, and we love sharing them.

bhworks for SPAR/GPRA/NOMS

Tired of wasting time trying to submit your grant program data to the Substance Abuse Mental Health Services Administration (SAMHSA) and other government agencies? Reduce data reporting from weeks to within 24 hours and minimize data errors with bhworks data collection and reporting tools.

Implementing Behavioral Health Screening in a Pediatric Emergency Department

Jessica Williams, lead social worker in charge of the behavioral health screening protocol at Nemours/Alfred I duPont Hospital for Children in Delaware, wants you to know that depressed kids don’t have “a look”. As “champion” of the screening protocol, Williams manages all staff who interact with bhworks in some way. With a little help from the hospital’s EHR analyst, and the collaborative efforts of the emergency department team, Williams helped to drastically increase the hospital’s screening numbers in just a few months. In the process, her and her team learned some valuable lessons about why we should be asking kids about their behavioral health.

Leveraging Peer Recovery Specialists to Support Vulnerable Populations

After realizing they were spending significant resources treating patients who were frequently admitted to the hospital with underlying behavioral health problems, Bon Secours Baltimore decided to pilot a program that leverages peer recovery specialists to better support this population. The peer recovery specialists use bhworks to screen patients for behavioral health and social risk factors, connecting them to primary care, housing assistance, transportation services, education and employment opportunities, and other resources that remove barriers to care.

Behavioral Health Integration in Primary Care

Behavioral health is becoming a part of the way our country looks at whole health, but it takes pioneers at the individual practice level to pave the way for real change. That’s why we were so excited to meet Susan Matthews through an initiative to increase behavioral health screening in primary care practices. When we approached her with bhworks, she was already in-tune with the needs of her rural Kentucky community, and she was ready to roll up her sleeves and get to work.
