The UPMC Health Plan Enrollment Department is committed to ensuring you have access to the documents, forms, and information you need to manage your account’s health benefits plan(s).  The Enrollment Contact Form, which currently is available via the UPMC Health Plan public website, is expected to move to Producer OnLine by April 30th, 2022.
