In a value-based care world, physician groups, ACOs, and hospitals are accountable for the entire patient journey. Therefore, their long-term success and financial viability are inherently tied to the performance of their network of post-acute care (PAC) providers. Likewise, as value-based care programs continue to dominate traditional fee-for-service care delivery, it is critical for skilled nursing facilities (SNFs) to establish themselves as essential providers to their healthcare partners. Now is the time to align acute and PAC value-based care initiatives to deliver better clinical and financial outcomes across the entire patient continuum.
