Please note that there are two options for completing this form: 

Option 1: Send this survey to everyone in your LEA who will be participating and have them update it.

Option 2: Complete the roster template included, and we will add your list to our tracking roster

The grant will require a full roster by the end of July, but if you would like to add one or two people now, please complete the survey and select “Filling out this form for myself” to add these contacts. You may complete the form again when the LEA is ready to upload its full roster of leaders, coaches and teachers. 

LEA Leaders participating in the Strong Foundations Planning and/or Strong Foundations Implementation Grant are provided with required and optional training and informational sessions to help prepare leaders for a successful grant process. This table pulls together all the events March – May in one place.
