Patricia Kappes took this picture of her mother, Mary Cook, when she was a resident of Van Duyn Nursing Home. Kappes said her mother had been sitting on the edge of her bed for hours, putting her at risk of falling. Cook died July 29.

Todd Barnes couldn’t see the bedsore on his backside, but he knew it was getting worse.

The staff at Van Duyn Nursing Home told him the opposite, he said. It was getting better. He didn’t need antibiotics or painkillers, he said he was told.
