5th Grade Party & T-Shirts
Please complete the 5th Grade Party sign up and provide a size for the end of year "31" T-Shirt for your 5th grader(s).    

Party Timing:
- 3:45-4:30 - Pizza @ BHE
- 4:30-5:00 - Travel to Urban Air
-5:00 - 7:00 - Urban Air Fun
- 7:00 - 7:30 - Travel to BHE

Please complete this form to: 
- Let us if your student(s) will be attending the 5th Grade Party
- Provide us with your student(s) T-Shirt Sizes
- Submit payment to BHE PTO by May 10th

In the next few weeks students will sign the "31" for the back of these shirts.  All students will need a T-Shirt as they will be worn at the the May 22nd Track & Field day and other end of year 5th grade events.
