Auburn School-Based Health Center Parent Survey
The Auburn School District could have the opportunity to establish a School-Based Health Center (SBHC) offering comprehensive health care to PK-12 students by a local healthcare organization. School-based Health Centers are typically staffed by nurse practitioners, coordinators, and mental health counselors, and offer services such as diagnosis and treatment of minor illnesses and injuries, management of chronic health conditions such as asthma, sports physicals, immunizations, reproductive health, and counseling for sadness and stress. Services are provided regardless of student’s ability to pay through local, state, and federal grants.
We are collecting parent input about the possible School-Based Health Center to evaluate students’ health care needs. All answers are anonymous. Please complete this survey by May 20th, 2022 .
