Epic Survey for FM Home for Dinner Initiative

This survey should take less than 5-10 minutes to complete. Its intention is to prioritize interventions of the FMCH Department's Epic Mastery Task Force for our Family Medicine Home for Dinner Initiative. Individual responses will be used for individual Epic coaching sessions (if you choose to participate) and will not be publicly released. We will share de-identified, aggregate results of the survey to inform our initiative. (Of note, this is Epic and outpatient-oriented but there will be support for other clinical circumstances)
