Care Coordination and Community Partnerships for Cancer Care in Critical Access Hospitals

Abstract

This brief presents data on care coordination and community partnerships for cancer care in Critical Access Hospitals (CAHs). Data come from a survey of 135 CAHs across the U.S. and provide specific data on provision of care coordination services and staff providing these services. Additionally, information is included related to partnerships between CAHs and community organizations for cancer initiatives.
