Physical Examination

Students registering for Activities must have a current physical examination form submitted for the Activities Office review before the student is authorized to participate in practice or tryouts. Physicals are valid for 12 months from the date of the exam and must be conducted by a New Jersey Physician. Click HERE to get a copy of the physical form.
