2021-2022 Club Sports Drivers Agreement

Each Club Sport member who drives a vehicle for club purposes must submit a copy of their insurance and drivers license and be added to the list of Club Sport Approved Drivers. The Club Sports Approved Drivers List allows us to receive any changed information (contact info, name) or violations (tickets, suspensions) to the club member's record for as long as he/she remains a member of a Club Sport. Please see the Club Sports website for further details and procedures.

I will IMMEDIATELY notify RecWell, through the Assistant Director for Intramural and Club Sports, if there is a change in the status of my driver's license (i.e. revoked, suspended, points added, etc.).

I understand that serving as a Club Sport driver is a large responsibility; if I am not comfortable with it, I understand that I should consider making alternate travel arrangements.

In addition, I warrant that the information I have provided is true and accurate, and falsification of any information may result in suspension or revocation of my membership in the RecWell Club Sports program. Each driver's personal car insurance coverage will be called upon in the event of property damage, personal injury, or accident.
