EIC Incident Reporting form

IF YOU NEED TO REPORT AN EMERGENCY, DO NOT USE THIS FORM. INSTEAD CALL THE DREXEL UNIVERSITY POLICE AT 215-895-2222 OR DIAL 911.

Drexel University is committed to fostering a safe and welcoming campus community for all students; in turn, it is the responsibility of each person within the Dragon community to join in creating an environment in which others can thrive without fear of harassment, discrimination, or sexual misconduct. As listed in our policies, anyone who has been the target of, witness to, or has knowledge of an incident of harassment, discrimination, or sexual misconduct may use this online form to make a report to the University. You can find our complete policies related to those incidents here on our website.

When reporting incidents of harassment, discrimination, or sexual misconduct through this form, please also note the following:

o  Your confidentiality and trust is very important to us. We will make every effort to respect your privacy. Please be aware, however, in certain circumstances involving safety or criminal action, we may not be able to guarantee complete anonymity. Regardless of the situations, matters are generally handled discretely and in conformity with University privacy policies, and information about individuals or situations will only be shared with individuals with a legitimate need to know. The University also has strong anti-retaliation policies that protect individuals who report incidents or cooperate in an investigation.

o   Anyone reporting an incident through this portal should provide either a phone number or an e-mail address so that we may contact you if there is a need for clarification or if additional questions or concerns arise. Also, if you want to receive a copy of your report, you will need to enter an email address in the top section. Please be aware that by not providing contact information in your report, you may limit our ability to address the concerns you have brought forth. If you require access to resources or support from our office but are unsure if you want to report an incident at this time, you can find more information by emailing us at eic@drexel.edu.

o   Drexel University’s primary concern is the health and safety of its students, which is why we created the Responsible Dragon Amnesty Policy (RDAP). The University is aware that Students are sometimes reluctant to seek medical attention in alcohol and drug related emergencies out of fear that they may face sanctions related to possessing or consuming alcohol or other drugs. Drexel University seeks to create a culture of trust and care paired with safety and responsibility while reducing any barriers that would prevent students from seeking assistance. RDAP provides amnesty from disciplinary action for Students or Student Organizations who seek assistance during an alcohol or drug related emergency.

o   Drexel University’s primary Drexel University provides access to confidential resources to our students. Students can access on campus medical care by calling the Student Health Center at 215.220.4700. Counseling services are available through the Counseling Center Monday through Friday, 8am-5pm ET at 215.895.1415. If you are currently in crisis, please contact our on-call counselor, available 24-hours a day at 215.416.3337.

o  Some experiences that are reported could fall under multiple protected classes. For example:

  • Experiences of antisemitism could fall within protected classes such as religion, ethnicity, or national origin.
  • Experiences of ableism could touch on disability, age, or veteran status.
  • Experiences of transphobia could fall within sex, sexual orientation, gender, gender identity and/or expression, depending on the specific conduct.

If you are unsure how to define your experience, please select "other" or "do not know" and you will have an opportunity to share more with a Case Manager.
