member. (*Recommendation cannot be from a family member).

· The application must be fully completed; incomplete applications will not be considered.
· The application deadline is May 13th, 2022.
· A copy of high school transcripts, ACT/SAT Test Scores and a copy of the SAR report from the FAFSA must accompany the application. (Application will be void if all items are not attached)
· The Scholarship Recommendation Form must be completed by a non-family member and returned with the application.
· By returning this application you agree to attend the Awards Banquet, May 17th at 6:00 pm, held at Fratelli’s Italian Restaurant, in Barboursville, WV
