LCS Intake Information Form
Labor Community Service Member Assistance Request Form is to be completed by the union member requesting assistance. This information is reviewed by our team who will follow up on the information submitted. This form does not guarantee assistance but we commit to supporting as many members as resources suffice.  Upon completion of this form please email Labor Community Services program manager Taro O'Sullivan at taro@lcs-la.org the following LCS intake form. Please be advised that members applying for our services are required to provide documentation such as but not limited to a form of Identification, rent agreements, rent receipts, utility bills, unemployment benefits, and pay stubs.
