1. You may print a Pre-participation - Physical Examination Form by selecting Download and Print tab at the top of the page or use the paper copy that we provided. Once you have selected the tab you will see the link to the Pre-participation - Physical Examination Form and will be able to download and print.

2. The Parent-Student-Athlete Handbook can be printed for your reference. The handbook includes important UIL Health and Safety information as well as guidelines for athletic participation in Life School. There will be a signature page within the Electronic Participation Forms stating that you have been provided and agree to follow all rules and guidelines within the handbook.

3. All other required paperwork will be done electronically. The parent/guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of the primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically.

4. Remember that to submit online appropriately, you and your child must fill out and sign in all required spaces. Forms will not be accepted electronically without all required information being completed.

5. In the upper right-hand corner, click Fill Out Electronic Forms. Then, select Texas under the STATE dropdown menu. Scroll down and click on LIFE SCHOOL. You will be directed to the Life School Rank One Sport Page. Once at this site, click on INSTRUCTIONS.

6. You can begin filling out information or you can click on REGISTER NOW and create an account. We strongly encourage you to create an account. Having a registered account allows you to update any information throughout the school year. To access the required athletic participation forms for Life School, hold your cursor over the Electronic Participation Forms tab. You will get a drop down list of all required forms (Emergency Liability Waiver, UIL/Athletic Pledge and the Athletic Fee payment).

7. To begin, click on the form name and fill out the information requested. The Parent/Guardian must have their student present along with the student’s birthdate to complete the forms. If you do not know the answer to a question, please answer the question with N/A, or NONE.

8. The online forms require both a parent/guardian and student signature. The parent/guardian and student must fill out the forms together.

9. To sign the document, click inside the signature box and hold your mouse down; this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an IPad, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus.

10. Once you have filled out and read the information on each page, make sure you hit the SUBMIT button at the bottom of the page.

11. After you have filled out all the forms you will need to click the Athletic Fee tab under the Electronic Participation Forms tab. You will then click the “Add to Cart” tab and complete the information. The athletic fee is mandatory for your child to participate in athletics.

12. The hard copy of your Pre-participation - Physical Examination Form must be turned in to your student’s campus athletic department. All other forms will be submitted electronically.

13. You will receive a confirmation email once the documents have been reviewed by Life School.
