IMPORTANT:

If you are adding dependents to your coverage you will need to bring legible copies of supporting documentation, such as: Copy of Social Security Card, Long Form Birth Certificate, Marriage License or page 1 of your federal tax return, etc. to your New Hire Benefit Enrollment Appointment. *Your enrollment coverages will not be fully processed without the required supporting documentation.*
