healthcare providers, farmers markets, and others in Lawton, Muskogee, and Miami

to provide fresh fruits and vegetables to individuals and families. The farmers markets have locally grown, affordable, fresh produce that is sold by easily accessible vendors.

how do i enroll?

On this website, click the green button in the top right corner of the menu tab titled "Enroll Here with 2023 Provider Referral Code." You will be prompted with a survey. Please complete the full survey and enter your referral code when prompted. Once the survey is complete, you will be enrolled in the program.
