This form will help us know how many people will be at the event so we can have enough food for everyone. There is also an option to sign the participation waiver electronically in advance of the event.

To submit your participation waiver electronically, please read the following and type your name as your signature (Or you can choose to sign your waiver upon arrival at the event).

I give my permission for the children listed below to participate in the 2023 Turkey Trot Family Fun Run sponsored by the St. John’s Lane Elementary School Parent-Teacher Association, Inc. (SJLES PTA). I, the undersigned parent or legal guardian, intending to be legally bound, do hereby for myself, my child(ren) listed below, and each of our heirs, executors, administrators and assigns, forever waive, release and discharge any and all rights, claims and actions for damages that I may have, or that may hereafter accrue to me against the SJLES PTA, including all unit, council and district organizations and all of their officers, directors, members, agents, contractors, and volunteers.

I attest and verify that I, and my child(ren) listed below, am/are physically fit and able to participate in this event and acknowledge that I am aware of the inherent risks in participating in an event of this type.

I Accept. Electronic Signature Agreement: By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
